Referral Guidelines for Medical and Psychological Evaluations for Affidavits:

• Fax completed referral form to Erica Hastings at 617.414.4796
• Specify type of affidavits needed based on your interactions with the client
• Make the appropriate referrals at least 3 months prior to when the affidavit is needed. Keep in mind that at times it may take several weeks to get a first appointment
• It is often helpful to print a map for the patient/client to assist them in navigating our large campus.

General Health Care Referral Guidelines:
